Submitter: Name of individual submitting the date.

Submitter’s E-mail Address:

Borden or Site Number: Alphanumeric designation for site, if applicable.

Site Name: The name of the site, if it has one.

Geographic Location: Country, province, territory or state; may contain information such as county, island, river, or drainage, if known.

Component: What area, feature, or horizon did the sample come from?

Provenience: Describe the context of the sample (i.e. site area, feature, unit, level).

Map Coordinates: If known. Decimal degrees preferred.

Lab Number: (e.g. Beta – 123456; AECV-1007; S-1000)

Field No.: The field/artifact catalogue number of the sample, designated by the excavator.

Material Dated: E.g. charcoal, wood, bone collagen, etc. (if bone collagen, include common name of taxon dated).

Taxon Dated: The scientific name of the taxon dated, if known.

Uncorrected Age: Measured age of sample, NOT corrected for isotopic fractionation.

Normalized (Corrected) Age: Conventional age of sample, CORRECTED for isotopic fractionation.

Delta C13: If available.

Note: Describe any specific issues that arose during the recovery, storage, or testing of the sample.

Significance: Why was this sample chosen for dating?

Suggested Age: Submitter’s estimated age (is the date consistent or inconsistent with other temporal indicators/ additional dates).

Associated Taxa: The scientific names of the taxa found in association with the sample.
